The practice

First, we listen.

Every treatment here begins the way it has for forty years: three fingers resting on the wrist. Before anything is prescribed, the pulse is read — vata, pitta and kapha, each moving with its own gait, telling the physician what the body has been doing and what it now needs.

Consultations are forty-five minutes and always in person. The pulse cannot be read through a screen, so we do not try.

The prescription comes from the reading — never from a menu.

Named for its number. Five actions, one protocol.

Pancha — five. Five classical cleansing actions, prescribed singly or in sequence to restore the balance of the five elements the body is built from. As each one passes below, a petal takes its place.

First

Vamana

Therapeutic emesis

The directed clearing of the chest and stomach — the classical seat of kapha. Prescribed for congestive and respiratory patterns, and only after days of preparation have loosened what must leave.

Second

Virechana

Therapeutic purgation

A precisely dosed herbal purgation that carries excess pitta out through the intestinal tract. The most frequently prescribed of the five, and gentler in experience than its name suggests.

Third

Basti

Medicated enema

Warm herbal oils and decoctions introduced where vata is governed. The classics call basti half of all medicine; it is the deepest-reaching of the five and the heart of most programs here.

Fourth

Nasya

Nasal administration

Medicated oils delivered through the nose — the door to the head. Prescribed for the senses, the sinuses, the voice and the mind, and paired with unhurried massage of the face and shoulders.

Fifth

Raktamokshana

Blood purification

The rarest of the five, used with great selectivity where the blood itself carries the imbalance. Its presence in the canon completes the system — five actions for five kinds of accumulation.

The residential program

The eight days.

Panchkarma is not a spa visit; it is a sequenced medical protocol. The arc below is representative — your own program is prescribed at the first pulse reading and adjusted daily. Programs run in Albuquerque and Zurich during visit periods, and at the Nagpur home clinic through the year.

Day 1

The reading

Pulse, history, and the prescription of your protocol. The program begins with listening.

Day 2

Snehana begins

Oleation, inside and out — warm medicated oils prepare the tissues to release what they hold.

Day 3

Abhyanga & swedana

Synchronized full-body massage followed by herbal steam. The body softens; circulation carries the oils deeper.

Day 4

Shirodhara

A continuous stream of warm oil to the forehead. The nervous system settles into stillness.

Day 5

The karma

Your prescribed cleansing action, precisely timed to the preparation that came before it.

Day 6

Completion & rest

The action concludes; the body is observed, warmed and fed simply while it recalibrates.

Day 7

Rasayana

Rebuilding — rejuvenative herbs, a graduated diet, and the daily routine you will carry home.

Day 8

Departure

Your home protocol in writing, and a follow-up plan with the doctors. The program ends; the practice continues.

The physicians

Four decades. Two doctors.

Dr. Sunil V. Joshi

Founder · M.D. (Ayurveda)

Trained at Government Ayurveda College, Nagpur, with a doctorate in the Charaka Samhita — the foundational text of Ayurvedic internal medicine. He opened Vinayak Panchkarma Chikitsalaya in Nagpur in 1985 and brought the practice to Albuquerque a decade later. Author of Ayurveda and Panchakarma: The Science of Healing and Rejuvenation, and for ten years clinical faculty at the Chopra Center.

Dr. Shalmali S. Joshi

Physician · Women's health & nutrition

Practicing panchkarma since 1987, with a particular focus on its application in gynecological and women's health, and on Ayurvedic nutrition as daily medicine. She designs the dietary arc of every residential program and leads the kitchen teaching that patients take home with them.

The lineage

The clinic the West trains at.

Vinayak is not an interpretation of Ayurveda for America; it is the Nagpur practice, continued in Albuquerque since 1995. Practitioners from the European Institute of Vedic Studies complete their clinical rotations at the Nagpur clinic, and Dr. Joshi has taught Ayurvedic medicine at institutions on three continents.

The practice travels.

The home clinic is in Nagpur, where the practice has run without interruption since 1985. Dr. Joshi brings it west on scheduled visits — consultation and program dates are announced for each visit, and the list hears first. Next: Albuquerque, 23 September – 4 October (Dr. Sunil V. Dr. Joshi is the architect of the Oneness Ayurveda Panchakarma Protocol at Sattva Campus in Southern India — a residential retreat where his three-phase clinical architecture, constitutional assessment and physician-certification standards govern every programme. Retreats are delivered by a physician team trained and certified under him, and every guest’s thirty-day follow-up consultation is with Dr. Joshi himself.
